Hospice Care
When Is It Time to Consider Hospice? If you have questions about hospice care, you're not alone. Understanding your options can bring clarity and peace of mind during a difficult time. Hospice care is designed for individuals who have been diagnosed with a terminal illness and whose physician believes they have a life expectancy of six months or less if the illness follows its expected course. The focus of hospice isn't on curing the illness - it's on providing comfort, managing symptoms, and enhancing quality oflife, while supporting both the patient and their loved ones through the end-of-life journey. Lake Circle of Care is here to provide information, answer your questions, and help you determine whether hospice care may be the right choice for you or your loved one. For more information, please call 707-263-6222.
Palliative Care
Palliative Care provides an added layer of support to help you manage the challenges that accompany serious illness. Your quality of life is more than your medical problems and treatments - yet a serious illness can make life more stressful. Our professional, compassionate Palliative Care team is here to help, so you can focus on what matters most.
Enhanced Care Management
(ECM)
Enhanced Care Management (ECM) provides compassionate, personalized support for eligible Medi-Cal members with complex health and social needs. This no-cost program connects you with a dedicated Care Manager who coordinates your care, helps you navigate the healthcare system, and links you to community resources that support your overall health and well-being.
